Service Description
This FOLLOWUP/ANNUAL breast thermal screening is to be scheduled AFTER the initial baseline screening that has been completed at Functional Wellness & Imaging in Medina, OH. This FOLLOWUP/ANNUAL breast thermogram includes eight images plus a comprehensive thermal report from the Interpreting Physician received within 10 business days. A 15 minute complimentary phone consult with a thermography technician is included with this screening to answer questions regarding your report. Price is $250 for this FOLLOWUP/ANNUAL breast screening session. $50 deposit, to be applied to cost of service, is required for booking. Balance of $200 due at check in. We accept cash, check or Visa/MC. A 3.5% service charge will be added if using a c/c. Thermography Screening is true prevention and meant to be used annually to monitor inflammation in the body. As part of our commitment to your privacy and the security of your information, we will securely maintain all of your thermal images for two years after your last appointment. After two years we will delete your information to comply with data protection policies.
Cancellation Policy
Your purchase is non-refundable and may not be transferred to another service. All fees shall be deemed to have been earned upon payment. Unless otherwise provided by law, you acknowledge that all sales are final and that we do not offer refunds for any portion of your payment. By registering online, you agree that you will not issue a chargeback for any payment made as you are fully aware. Out of respect for staff and other clients, If you are more than 10 minutes late for your service, we will not be able to accommodate you. We schedule a specific amount of time to dedicate to you and your health.
