Service Description
Take control of your health with this personalized session with an FDN-Practitioner designed to support your unique needs and lifestyle, helping you to achieve optimal health and wellness. With Carole's expert guidance, you can make lasting changes to your well-being and live your best life. This telehealth session will include review of a basic intake form that will be emailed once you register. This needs to be filled out and returned via email 7 days prior to appointment. This intake form is how we begin to find the WHY! Together with Carole, you'll look at symptoms plus ONE of your recent labs of your choice. A lab that you have already completed. We'll identify areas for improvement and develop a plan that works best for you! Keep in mind, although Carole would love to include all of your lab work, this initial appt. covers review of one lab, that you have already completed and need clarification on how this relates to any symptoms that you may currently have. This lab needs to be emailed with the assessment form 7 days prior to appt. Schedule your session today and start your journey to a healthier you!!

Your purchase is non-refundable and may not be transferred to another service. All fees shall be deemed to have been earned upon payment. Unless otherwise provided by law, you acknowledge that all sales are final and that we do not offer refunds for any portion of your payment. By registering online, you agree that you will not issue a chargeback for any payment made as you are fully aware. Out of respect for staff and other clients, If you are more than 10 minutes late for your service, we will not be able to accommodate you. We schedule a specific amount of time to dedicate to you and your health.
