Is thermography covered by insurance?  NO - Unfortunately insurance does not cover thermography.  It is a self pay screening but GRANT MONEY IS NOW AVAILABLE!

You will be asked to provide a copy of your check or credit card statement showing that you paid for the thermogram up front along with copy of the receipt from our office. Functional Wellness & Imaging will provide you with a receipt. This is your responsibility. Our office only provides the receipt after the screening is complete. We DO NOT apply for you. You are required to pay in full for the thermal screening and get reimbursed from UBCF. Our office DOES NOT do that for you!

 

The "GRANT" for Thermography is available thru The United Breast Cancer Foundation.  You will be granted up to $200 for answering 7 questions on this simple application. See example questions below:


1. Name

2. Email

3. Phone

4. How many people in household?

5. Annual Income? (Be honest - -They do not ask for verification.)

6. Can you pay your provider at the time of service?(You pay in full for the thermography scan and UBCF reimburses you)

7. Donate $5 to the United Breast Cancer Foundation. (To be eligible you must donate $5 to help other women save money)You will be eligible for a reimbursement check mailed to you for up to $200.


We don't know how long this will be available --- get your application in ASAP! As many of you know, this "GRANT" has not been available for the last 2 years so apply NOW!


After you have received an email confirmation confirming the amount of monies awarded, you will then make your appointment on our website at functionalwellnessandimaging.com


This "Grant" may be used for "men" or "women" and for "breast scans" or "full body scans".

Program Guidelines

  • Open to women and men.

  •  You must apply before having each scan taken and have received the confirmation email from the UBCF before having your scan. Otherwise you are ineligible if you don't have the confirmation email.

  • Applicants may receive a screening of their choice from a provider of their choice. Thermography is covered!!

  • UBCF assists with base-line initial thermography, however, the 2nd screening of a baseline is covered at a lesser amount.

  • UBCF will not pay for thermography screenings that have taken place prior to an application being submitted, approved and a Breast Screening Grant Award provided to the applicant.

  • UBCF reimburses the applicant for the cost of the thermogram screening (payment is no greater than the Breast Screening Grant Award).

  • 60-day approval period – Once awarded a Breast Screening Grant, the applicant has sixty (60) days from the date of the Grant Award to have their thermogram breast screening completed AND to submit the original receipt to UBCF for reimbursement. If UBCF receives the receipt after the 60-day approval period, the Grant Award will expire.

  • The grant can be used towards a full body scan, since the breast are included in the scan.

  • It takes appx. 8-10 days to receive a confirmation email with the amount you have been awarded.  So keep checking your email.
